I declare that I have, of my own choice, shared through the website intra-oral photographs, radiographic images and information concerning my state of health; That I understand this data to be special category personal data within the scope of Article 6 of Law No. 6698, and that I give my explicit consent to its processing solely for the purposes of assessing my request, providing me with preliminary information and planning the appointment process, for the period stated in the disclosure notice; That I am not obliged to give this consent, and that if I do not, I can still create an appointment request and reach the clinic by telephone; That I may withdraw my consent at any time, and that processing of this data will cease from the moment I withdraw it; That an assessment made through the website does not constitute a diagnosis and does not replace a clinical examination — and I accept the above.
